A program for people 65 years and over that helps link seniors living at home with health and social services. Available to residents in Lanark Highlands, Carleton Place and Mississippi Mills.

Are you a senior living at home? Or is someone you care about a senior living at home?

Our Primary Care Outreach to Seniors program works collaboratively to support seniors.

Our Primary Care Outreach (PCO) team includes a registered nurse and a community health worker who provide ongoing support to seniors, focusing on what is important to them. Through a shared-care approach, we work with other healthcare professionals, geriatric partners, community agencies and families. The team provides support with care and links seniors to community resources which assist with various needs and help maintain health and independence.

We strive to improve wellness and to reduce isolation, emergency department visits and hospital admissions. The program is offered at no cost to the client.

Referrals are accepted from any source for eligible seniors aged 65 and older.

This program is run in partnership with South East Ottawa CHC.
