ConnectWell Community Health is seeking an individual who is client centered and works collaboratively within a multidisciplinary team 1.0FTE for 35 hours per week. This is a full-time, ongoing position that includes a comprehensive benefit package with HOOPP.


Position Summary:
The System Navigator/Intensive Case Management for Youth is a dual role that plays an important part in ConnectWell’s approach to serving clients with multifaceted emotional, physical health, and social challenges. This position works closely with the mental health therapists/counselors and the primary care team at Lanark and collaborates with the broader social services and health community.

The System Navigator collaborates with the Lanark team to help adult clients navigate to services within ConnectWell and the broader community. They are responsible to complete comprehensive adult client intake assessments, enabling appropriate referrals and facilitating connections for individuals to community services and supports.

The Intensive Case Management Worker (ICMW) portion of the position assists youth with mental health needs and assisting in the integration from child and adolescent mental health services to adult mental health services. The ICMW is responsible to assess the individual needs of the youth and support them in mental health wellness. The ICMW facilitates the youth to connect to resources within ConnectWell and/or in the broader health care community.

Qualifications:

• A Baccalaureate degree in Social Work, Counseling, Psychology, Social Science or Health related field (or equivalent education and experience).
• Three to five years’ experience in the mental health field, providing community-based mental health services including crisis intervention, group work, referral and advocacy for clients with complex issues.
• Experience in case management, completing assessments, interviewing and assessing client needs, counseling for substance abuse and goal setting.
• Experience with application of a variety of screening and assessment procedures and tools
• Experience in engaging people with significant barriers to accessing health, social services and housing.
• Experience working with difficult client behaviors, working within a harm reduction framework.
• Experience in service integration and transition from child to adult mental health services
• Attention to detail, ability to prioritize and time management skills
• Excellent interpersonal, oral and written communication

Reporting Relationship: The successful candidate will report to the Director of Mental Health Services and Connections.
