A program for current clients who are living with or at risk of cardiovascular disease, such as high blood pressure and heart attacks. Available in Lanark and Renfrew Counties.

Do you have high blood pressure?
Have you experienced a heart attack or stroke?
Whether you are at risk for heart-related illness or you have experienced significant heart or cardiovascular problems, we are here to help you reduce your risk of future heart problems.
If you have one or more risk factors for heart attack or stroke, we can work with you one-on-one to coach you on heart healthy skills, including:
- Learn to monitor your blood pressure at home
- Learn about the effects of sodium (table salt) on your heart
- Set lifestyle goals, which might include nutrition, physical activity and stress factors, to reduce your risk of heart problems
- Offer Quit Smoking support
- Learn about heart-health, stress management, the effects of improved physical activity and healthy eating
In addition to our one-to-one support for clients, we also have many group programs available to all community members that can help you reach your health goals. Chronic Disease Rehabilitation Program for people with Cardiac or Chronic Lung Disease
ConnectWell Community Health- Lung Health Program also provides a chronic disease rehabilitation program. It is an exercise program designed for people with COPD or with cardiac disease. This program is offered in partnership with the University of Ottawa Heart Institute.

Our aim is to help decrease your symptoms and increase your ability to function in daily life.
We celebrate our accomplishments. We support and celebrate each other when we are happy, sad or hurting. Our classes are so important to us. It gets us moving and out of the house. Our monthly information classes are invaluable. I never fail to learn at each session.
A few years ago, when I first started the Program I stated “I feel like I have joined the land of the living” Through this Program we are meeting others, learning to positively cope with the disease, becoming physically stronger as well as have the security of knowing people who care who are professional, compassionate and there to support me when needed is what I sincerely thank you for.
