When you become a client, you become a client of ConnectWell Community Health Centre. Based on your health profile, we will match you with a provider. Our inter-disciplinary team of health care providers and allied health staff will work with you to help you meet your health care goals.
Our doctors are family physicians who have an expert knowledge of a wide range of common health problems in all age groups and of less common but life threatening and treatable emergencies. They see clients with chronic diseases, emotional difficulties, acute disorders and complex health conditions. They may provide palliative care to people with terminal illnesses. They provide care for people here at the health centre, in hospital and on and on a case-by-case basis, they will see clients in their home. Our physicians work with the full inter-disciplinary team.
Our Nurse Practitioners (NPs) are Registered Nurses with advanced university education and experience in primary care. Nurse Practitioners offer a unique blend of nursing, medical knowledge and skills. Nurse Practitioners provide a complete spectrum from the beginning of life to the end of life; from wellness care to chronic and complex illness care. Nurse Practitioners assess and diagnose illness, order diagnostic tests and x-rays and prescribe treatment and medication. They also refer clients to specialists when needed. Nurse Practitioner’s offer counseling and education. Our Nurse Practitioner’s work with the full inter-disciplinary team.
