ConnectWell Community Health is seeking a Community Worker – System Navigation Access & Partnership (SNAP) Project for 10.5 hrs/week, contract position to begin in October 2023 until July 31, 2024.

Position Summary:

The Community Worker will provide leadership to ensure the project key activities are completed to meet defined outcomes and achieve project impacts by the grant project deadlines. Key activities include increasing the reach of the System Navigation services to community members, building on social inclusion project activities and testing an asset-based community development model in a specified rural community.

Qualifications:

• Post-secondary education (Bachelor’s degree/diploma in health or social sciences) or equivalent combination of training and experience
• Minimum two years community development or community health experience
• Experience engaging and supporting diverse, adult groups in community development, advocacy or other community building activities and projects
• Demonstrated ability to work collaboratively in a multi-disciplinary setting, with clients, community members, volunteers, staff, and partner agencies
• Excellent communication and group facilitation skills
• Knowledge of health promotion principles, community development and social prescribing concepts
• Excellent communication skills, both oral and written.
• Excellent organizational and interpersonal skills.
• Experience and understanding of rural communities and rural health needs.

Reporting Relationship: The successful candidate will report to the Director, Renfrew County.
