ConnectWell Community Health is an integrated organization governed by one community-based Board of Directors, with a Senior Management Team, and are guided by core set of: Beliefs, Vision, and Mission; policies and procedures that set standards for accessibility, privacy, health and Safety etc.; strategic priorities; collaborative team structure; and service commitments.

ConnectWell previously operated as Lanark Renfrew Health & Community Services (LRHCS) that included three parts: Lanark Community Programs, North Lanark Community Health Centre, and Whitewater Bromley Community Health Centre.

The name change occurred as Lanark Renfrew Health & Community Services became an increasingly more integrated organization since five organizations voluntarily joined together under one Board of Directors, one organization and one senior leadership team almost 20 years ago. With services ranging from primary health care to autism services, ConnectWell’s programs and services will remain the same across its regional landscape serving Renfrew, Lanark and Leeds and Grenville counties. The name, ConnectWell, bridges together two words that define the organization’s core values and mission.

The ConnectWell Community Health Centres, located in Lanark, Cobden, Beachburg and Eganville, provide a wide range of primary health care, community development initiatives and health promotion programs to improve the health and well-being of individuals and communities.

ConnectWell also provides services to children and families, especially those made more vulnerable by developmental, physical or economic limitations.
