ConnectWell Community Health is seeking a Nurse Practitioner for between 28 hrs/week (0.8 FTE) to 35 hrs/week (1.0 FTE). This is an ongoing position and provides a comprehensive benefit package, HOOPP Pension and no weekend work.

Position Summary:
The Nurse Practitioner (NP) works as member of an interdisciplinary team providing primary care to clients rostered to the NP at the CHC in Beachburg/Cobden/Eganville (28 hrs/week). This position can also provide primary care to youth age 12 to 25 at the Youth Wellness Hub in Pembroke (7 hrs/week).

Qualifications:
• RN (EC) license/NP Primary Health Care Certificate
• Current and unrestricted registration as an RN (EC) with the College of Nurses of Ontario
• Three years experience in the nurse practitioner role in a primary care setting or community preferred
• Demonstrated ability to work effectively in an inter-disciplinary team
• Strong clinical skills
• Proven commitment to and knowledge of community-based health care, preferably in a rural community and with disadvantaged population groups
• Experience in the development and delivery of health education/promotion programs.
• Ability to work flexible hours, if required
• Valid driver’s license and use of a vehicle
• Attention to detail, ability to prioritize and time management skills
• Excellent interpersonal, oral and written communication

Reporting Relationship: The successful candidate will report to Director, Renfrew County.
