Our community paramedics work as a link between our doctors and clients in Renfrew County.
Community Paramedicine programs are designed to serve the needs of their community. In collaboration with existing services, The Renfrew County Community Paramedics acts as an extension of primary care. Paramedics are well positioned to identify and bridge gaps in the health care system to provide patient centred case. The basis of the program is to be accessible, proactive, responsive and safe.
Some programs offered by the Renfrew County Paramedics:
Renfrew County Virtual Triage and Assessment Centre (RC VTAC) : provides virtual access to primary care. Community paramedics work closely with primary care to perform clinical assessment and treatment in the home or virtually.
Remote Patient Monitoring: leveraging technology to remotely monitor patients with acute or chronic illness to help keep them well at home. Community paramedics provide timely monitoring assessment and treatment.
