ConnectWell Community Health is seeking a Caseworker to join our team.  This is a full-time, 35 hrs/week position.  This position provides a comprehensive benefit package, including Healthcare of Ontario Pension Plan (HOOPP).

 Position Summary:

The Respite Caseworker is responsible for coordinating respite support plans. They work with the Respite Team to provide practical help, connect families to services, and support them in building care networks.

Qualifications:

  • Diploma or Degree in one of the Social Sciences or Health related field or combined education and experience
  • Substantial and proven experience in supporting children and adults with special needs. As well as Strong commitment to and knowledge of community-based resources for individuals with intellectual disabilities, preferably in a rural setting
  • Knowledge of programs and services that improve the quality of life for individuals with physical and/or intellectual disabilities
  • Ability to work a flexible schedule, based on client(s) and program needs as required. May include evenings & weekends

Reporting Relationship: The successful candidate will report to the Program Manager, Client Care

Salary Range:  $53,633 to $63,098
