ConnectWell Community Health is seeking a Registered Practical Nurse (RPN) for 21 hours per week (0.6 FTE). This is a contract position from May 2024 until January 2025. This position is eligible for participation in HOOPP.

Position Summary:

The Primary Care Registered Practical Nurse (RPN) promotes the health of individuals, families and the community we serve. The Primary Care RPN is an important member of the inter-professional care team and practices nursing within a team environment to their full scope of practice. A RPN works in stable circumstances where there is predictability in the client’s care requirements and health outcomes, and where there is a limited risk for negative outcomes associated with the nursing care provided.

Qualifications:
• Registered Practical Nursing Diploma
• Current and unrestricted registration with the College of Nurses of Ontario
• Three years of nursing experience preferably working in a community setting
• Demonstrated ability to work effectively in an inter-disciplinary team
• Attention to detail, ability to prioritize and time management skills
• Excellent interpersonal, oral and written communication

Reporting Relationship: The successful candidate will report to the Director, Primary Care.
