CHCs offer a wide range of services with a team approach with a variety of professionals, including:
Admin Team Members, Primary Care Providers (Doctors and Nurse Practitioners), Nurses, Pharmacists, Social Workers, Respiratory Therapists, Community Health Workers, System Navigators, Health Promoters and Dietitians
This team provides primary health care, clinical treatment, and additional services such as group programming such as fitness, cooking classes, chronic disease management courses, and advance care planning sessions, counselling and community development activities
