Intensive Case Management for Youth

The Intensive Case Management Worker (ICMW) helps youth navigate challenges and build useful skills The ICMW works closely with individual youth in a client-centered manner and supports them in mental wellness as well as facilitating connections to resources within ConnectWell and/or in the broader health care community.  We provide welcoming, inclusive and non-judgmental mental health and wellness support to help meet the individual needs of those we work with.
