ConnectWell Community Health primary care clients can access free nicotine replacement therapy (NRT), if eligible, through the Smoking Treatment for Ontario Patients program (STOP). This program is in partnership with the Canadian Association of Mental Health (CAMH) and the Ottawa Model for Smoking Cessation (OMSC) in partnership with the University of Ottawa Heart Institute.
Nicotine Replacement Therapy (NRT) includes patches, gum, lozenge, and/or inhalers.
ConnectWell Community Health- Primary Care Team will work with clients to become smoke free. Clients can access up to 26 weeks of NRT combined with individual counselling and support. NRT is provided through the STOP program.
