Our System Navigator works closely with our mental health and primary care staff to help people navigate services within ConnectWell and the broader community.
Speaking with the system navigator can help assist you in finding programs and services that are available at ConnectWell and within the community to support your needs. The system navigator service is for non-urgent issues and provided on a first-come first-served basis.
How to access the System Navigator
You are encouraged to self-refer however, referrals can also be made on behalf of someone else, providing the person has given consent for the System Navigator to contact them. This would include referrals made by both internal ConnectWell and external health and social services providers.
