Are you a senior living at home? Or is someone you care about a senior living at home?
The Primary Care Outreach to Seniors (PCO) team serves seniors aged 65+.The team visits seniors in their homes, in the community, and in the office. PCO helps seniors to maintain their health, community connection and safety at home. The team includes a registered nurse and a community health worker who collaborate with other health care staff and community agencies.
Services Offered include:
- Help with medication review
- Help with forms
- Accompaniment to medical appointments as appropriate
- Links to supportive community and social programs
- Chronic illness management
- Help accessing housing support services
