Primary Care – Outreach to Seniors

A program for people aged 65 years and over. Available to residents in Lanark Highlands, Carleton Place and Mississippi Mills.

The Primary Care Outreach to Seniors (PCO) team serves seniors aged 65+.The team visits seniors in their homes, in the community, and in the office.  PCO helps seniors to maintain their health, community connection and safety at home.  The team includes a registered nurse and a community health worker who collaborate with other health care staff and community agencies.

Services Offered include:

• Help with medication review
• Help with forms
• Accompaniment to medical appointments as appropriate
• Links to supportive community and social programs
• Chronic illness management
• Help accessing housing support services
