ConnectWell Community Health provides a range of services to support individuals from birth through adulthood. This includes service to children and families, especially those made more vulnerable by developmental, physical or economic limitations. Services may be provided at one of our locations in Lanark or Leeds and Grenville, in a client’s home, or other community location.
