Knowing who to reach out to and seek help from can be challenging when navigating several complex needs and services.

Should your family be seeking support in coordinating the services you are working with, we may be able to support through Coordinated Service Planning.

In Coordinated Service Planning, we support families in bringing a child’s team together to work collaboratively on child and family centered goals, develop a Single Plan of Care to be used across environments and support the monitoring of goals as we move along in the process.

You are not alone. We are here to help you navigate the process. We have walked many families just like yours through the system. Here is sampling of what we can do for you:
