Family Foundational Services

Foundational Family Services are available to families/caregivers of children and youth who are registered with the Ontario Autism Program.

 At ConnectWell Community Health Carleton Place Site, we have a multidisciplinary team with over 40 years of experience working with children and youth with Autism Spectrum Disorder and their families. We offer a  variety of education opportunities on a wide range of topics for parents/caregivers. Foundational Family Services will provide you with tools and strategies to support your child or youth in the areas of daily routines, toilet training, communication, behaviour and returning to the school environment. Currently, our workshop sessions are taking place virtually using Zoom (PHIPA – Personal Health Information Protection Act approved). Following the workshops, you may have the option to schedule a 1:1 consultation session to discuss your specific goals with one of our experienced and well trained autism services team.

Virtual Consultation Clinics  – January 19th, February 9th. March 23rd

Families can contact Chelsea at 613-257-7121 ext. 3200 (or click on the link below to register) to consultation (up to 1 hour in length) between 9am-3pm with one of our team of Professionals (may include ABA/Behaviour specialist, Speech Language Pathologist, Occupational Therapist). This is an individual zoom appointment that will allow you to discuss a specific skill or area of priority for your child and or family. Please have this identified when booking your appointment.

Virtual Psychological Consultation Clinic – February 17th

Families can contact Chelsea at 613-257-7121 ext. 3200 (or click on the link below to register) to book a consultation with the Psychologist & behavioural consultant (up to 1 hour) between 9am – 3pm. This individual 1:1 Zoom appointment will allow you to discuss your questions regarding:

  • Your child’s behavioural needs
  • Psychological testing of children with special needs
  • Your child’s diagnosis (e.g. Intellectual Disability, Autism Spectrum Disorder, Global Developmental Delay).

Please have your questions/interests identified when booking your appointment. Please note that neither Psychological assessment nor a diagnosis will be provided during this brief consultation.

**NEW** Virtual Counselling Clinic – March 8th

Families can schedule (1-1.5 hour) counselling appointment between 9am-3pm with Clinical Counsellor.
This clinic day will provide you with an opportunity to speak with someone about your emotional health such as, dealing with stress or anxiety, relationships with others and conflict resolution to help enhance your own personal & family wellness

ABCs of Behaviour

Knowledge of the ABCs of behaviour is an integral part of understanding and managing your child’s behaviour. Functions of Behaviour

Do you find yourself questioning WHY your child behaves the way they do? This workshop will help parents to understand the function of their child’s interfering behaviour and learn about strategies to help that correspond with that function. *It is recommended that participants also register for our ABCs Workshop

Promoting Independence: Activities of daily living

Helping our children to become as autonomous as possible can be a rewarding yet demanding part of parenting. This workshop will help parents and caregivers of children with ASD learn how to select & teach meaningful activities of daily living in order to promote greater independence.

Preparing your child for Kindergarten

This workshop aims to help parents and caregivers explore strategies to assist their child to be ready for entry to Kindergarten. We will discuss what steps you can take at home and in your community to prepare your child for Kindergarten including focus on school readiness skills

De-escalation tips for managing escalating behaviour

This workshop focuses primarily on the prevention of escalating behavior. You will learn about recommended strategies from the Crisis Prevention Institute to assist in the prevention of and diffusion of those behaviours Note: This is not a CPI certified course.

Navigating Special Education Services

Understanding the special education system in Ontario can sometimes be a challenging & complex task for families. This workshop will introduce to participants the many processes & procedures of specific special
education services in Ontario. Parents will feel more confident to work with their child’s special education team to
effectively address needs for their child now and in the future.

Strategies for Smoother appointments

This workshop will provide Information to caregivers to help their child/youth attend successful self-care appointments (ie dentist, bloodwork, doctors visits). We will cover how to ease anxiety, prepare your child in
advance, and communicate with your healthcare provider around your child’s needs, and select tools to help
support on the day of the appointment.

Introduction to Respite Services in Lanark County

Families of children with special needs often require a number of services to enhance the health, well-being and life quality of their child and family. Respite care is one such service. ConnectWell staff will inform you of the different types of respite services, discuss how to go about hiring your own respite worker, and steps to apply for funding.

Disclosing your Child’s Diagnosis

In this workshop, we will discuss how to start the conversation with your child about their Diagnosis. We will aim to offer dialogue to explain and give your child a better understanding so that they can ask for what they need. We will provide resources to assist & guide you.

Puberty & ASD

This workshop is for families with a teen with ASD who is entering or experiencing the onset of puberty.This workshop will support parents as they help their teen understand the developmental changes they are going through. Parents will be provided with resources to further support their child’s needs during this time of change.

Transitions to Adulthood

This workshop is designed to help you develop a plan to address the steps that may lay ahead as your child transitions to an adult. By attending this workshop, you will become aware of changes in funding and services as well as common adult transitions that can require further attention as you move through
them.
